Xigduo and Ketoacidosis
Xigduo XR combines dapagliflozin (Farxiga) with metformin to help adults with type-2 diabetes control blood-sugar levels. Dapagliflozin belongs to a new class of diabetes drugs that work by helping the kidney eliminate excess sugar in urine. This class is known as sodium glucose co-transporter-2 (SGLT2) inhibitors.
In May 2015, the FDA warned that SGLT2 inhibitors were associated with 20 reports of diabetic ketoacidosis in their first year on the market. Soon afterward, the European Medicines Agency (EMA) launched their own investigation and said 101 cases had been reported worldwide.
Health officials advised seeking emergency medical treatment for symptoms like nausea, vomiting, abdominal pain, confusion, unusual fatigue, or breathing problems.
Another concern was that many patients had to be hospitalized. In some cases, their blood-sugar levels were normal or only mildly elevated, and symptoms were also mild. Experts were concerned that these factors could result in patients delaying seeking treatment until ketoacidosis was very severe.
