FDA Safety Warning

In December 2015, the FDA issued a Safety Communication to warn about the risk of pyelonephritis from diabetes drugs in the SGLT2 inhibitor class.

The agency received 19 reports of patients who developed life-threatening blood infections (urosepsis) and kidney infections (pyelonephritis) that started as urinary tract infections. All 19 patients were hospitalized and some required dialysis or intensive care.

SGLT2 Inhibitor Diabetes Drugs

What is the problem?

Diabetes drugs in the SGLT2 inhibitor class were first approved in March 2013. They control blood-sugar levels by forcing the kidneys to remove more sugar from the body in urine.

Unfortunately, excessively frequent and sugary urine significantly increases the risk of urinary tract infections (UTIs) and vaginal yeast infections in women. These infections sometimes spread from the bladder to the kidneys and cause pyelonephritis.

What is Pyelonephritis?

Pyelonephritis is a kidney infection. It can be caused by many types of bacteria or viruses, but most common cause is E. coli that travels into the kidneys from the bladder.

Warning Signs & Symptoms

  • Fever
  • Nausea, vomiting
  • Pain in the back, side, or groin area
  • Chills
  • Frequent and painful urination

Complications

Pyelonephritis can cause permanent scarring in the kidneys, which makes it harder for the kidneys to do their job. This can cause complications like high blood pressure, Chronic Kidney Disease (CKD), and kidney failure. In rare cases, bacteria enters the bloodstream and causes sepsis, which is a life-threatening whole-body reaction to toxins in the blood. Patients can go into “septic shock,” in which blood pressure plummets and organs fail.

Diagnosis & Treatment

Treatment begins with a urine culture to identify the type of bacteria that is causing the infection. Doctors will prescribe the most appropriate antibiotic, which is usually taken for 6 weeks. For severe infections, patients may need to be hospitalized to receive intravenous (IV) antibiotics, fluids, and other medications.
