Frozen Shakes Linked to Deadly Listeria Outbreak
In February 2025, health officials linked a deadly outbreak of Listeria to frozen shakes that were used at nursing homes, long-term care facilities, and hospitals for supplemental nutrition.

How Many People Got Sick?
Five illnesses and at least 1 death have been reported in Texas, making it the hardest-hit state in the outbreak of Listeria linked to ReadyCare and Sysco Imperial Frozen Shakes, according to the CDC Outbreak Investigation.
As of February 21, 2025, at least 38 people have been infected with the outbreak strain of Listeria monocytogenes. Of those people, 37 were hospitalized, and 12 people died. The illnesses were reported between August 2018 and January 2025.
This outbreak is particularly long-running and severe, with 12 deaths reported in 10 states: California, Illinois, Indiana, Michigan, Minnesota, North Carolina, New York, Tennessee, Texas, and Washington.
How Did Investigators Link Outbreak to Frozen Shakes?
Outbreaks of Listeria in long-term care facilities have been under investigation for years. Officials knew the cases were related because the victims were all infected with the same strain of Listeria, but they did not know what food was causing illnesses.
The CDC said it investigated this outbreak in 2018, 2021, and 2023, but did not have enough information to pinpoint the source.
In October 2024, yet another cluster of Listeria illnesses was reported in a long-term care facility, with 6 people infected. Frozen shakes were identified as a “product of interest” and inspectors visited the Prairie Farms manufacturing facility in Fort Wayne, Indiana.
The inspectors discovered the outbreak strain of Listeria in environmental testing at the facility, which confirmed that the frozen shakes were the likely source of this outbreak.
Lyons Magnus LLC Announces Recall for Frozen Shakes
On February 22, 2025, Lyons Magnus LLC announced a recall for Lyons ReadyCare and Sysco Imperial Frozen Nutrition Shakes.
The recalled products were sold frozen, but Listeria can survive in frozen foods and grow at refrigeration temperatures.
The frozen shakes were sold in 4-oz. cartons in flavors like Strawberry, Vanilla, and Chocolate. They were not sold in retail stores, but instead distributed to healthcare facilities nationwide, such as:
- Hospitals
- Nursing homes
- Long-term care facilities
- People with difficulty swallowing
- Other facilities
Who Used These Nutritional Shakes?
Lyons ReadyCare and Sysco Imperial Frozen Shakes are thickened liquids that are used for high-calorie, protein-rich diets — especially for older adults and others who have difficulty swallowing (dysphagia). The products are also used for supplemental nutrition, preventing unintended weight-loss, “soft food” diets, and digestive health.
Symptoms of Listeria Infection
Listeria monocytogenes is a bacteria that can cause severe, life-threatening invasive infections. People over 65 years old have a higher risk of complications, especially people with vulnerable immune systems. The symptoms of a Listeria infection can appear soon after eating contaminated food, but it can take up to 10 weeks.
