Farxiga and Ketoacidosis
Farxiga helps prevent hyperglycemia by inhibiting a protein in the kidneys called sodium-glucose cotransporter-2 (SGLT2). This causes the kidneys to eliminate sugar from the body in the urine, which helps lower blood-sugar levels.
In March 2015, the FDA issued a Safety Communication after receiving 20 reports of ketoacidosis in patients on SGLT2 inhibitors.
Experts are concerned that Farxiga patients with ketoacidosis may not be diagnosed in the early stages of the disease, which might increase the risk of life-threatening complications.
Ketoacidosis most often occurs in type-1 diabetics with very high blood-sugar, but Farxiga is a type-2 diabetes drug and victims may have normal blood-sugar levels. Therefore, patients on Farxiga should be vigilant for symptoms of ketoacidosis and seek medical attention immediately.
Symptoms of ketoacidosis from Farxiga may include:
- Difficulty breathing
- Nausea
- Vomiting
- Abdominal pain
- Confusion
- Unusual fatigue or sleepiness
What is Ketoacidosis?
Diabetic ketoacidosis (DKA) is a serious complication of diabetes that occurs when the body cannot use glucose (sugar) in the bloodstream because there is not enough insulin. It starts breaking down fat cells, which produces acidic wastes called ketones. High levels of ketones can poison the body.
Illnesses May Precede Ketoacidosis
Soon after the FDA warning for Farxiga and ketoacidosis, health officials in Europe and Australia issued similar warnings.
According to Australian health officials, sudden illness occurred just before the patient developed ketoacidosis:
“In some of the reported cases, just before or at the same time as the DKA occurred, patients experienced acute illness (such as, urinary tract infection, urosepsis, gastroenteritis, influenza, trauma or surgery), reduced caloric or fluid intake, and/or reduced insulin dose.”
