Wegovy Linked to Blindness, Vision Loss From Rare Eye Condition

In July 2024, the popular weight-loss and diabetes drugs Wegovy and Ozempic (semaglutide) were linked to an increased risk of blindness and vision loss due to a rare but severe eye condition called NAION.

What is the Risk?

Semaglutide, the active ingredient in Wegovy, was linked to a 4.28-times higher risk of an eye condition called Non-Arteritic Anterior Ischemic Optic Neuropathy (NAION), which causes sudden blindness.

Study Links Semaglutide and Blinding Eye Disorder

Patients with type-2 diabetes who took semaglutide were more likely to be diagnosed with NAION compared to people who were prescribed a non GLP-1 drug, according to the study published by JAMA Ophthalmology by researchers from Massachusetts Eye and Ear in Boston, MA.

What is NAION?

NAION occurs when the optic nerve loses its blood supply, which is why it is sometimes called an eye stroke. This causes tissue damage in the optic nerve, resulting in sudden vision loss and partial blindness. There is no cure for NAION, although the loss of vision may worsen over the next few weeks.

What Happens?

NAION is more likely to occur in people with risk-factors for circulatory problems, such as diabetes, cardiovascular disease, or high blood pressure. The condition is rare, estimated to occur in only about 6,000 people in the U.S. every year, but it can potentially result in blindness.

NAION: Sudden Painless Vision Loss In One Eye

In some reports, patients reported waking up in the morning and noticing significant vision loss in one eye, without pain, that did not get better. Because sleeping naturally lowers blood pressure, vision loss due to NAION may be linked to optic nerve tissue damage as a result of not having an adequate supply of blood or oxygen during the night.

Long-Term Prognosis for Vision Loss

NAION may involve only part of the optic nerve, which is why some people with this condition only suffer from partial vision loss. Patients describe it as a darkened area or “shadow” over the upper or lower portion of the visual field. Over time, optic nerve swelling due to tissue damage will improve, but the loss of vision does not get better.

• Blindness in one eye

• “Shadow” on upper or lower half of vision in one eye
• Sudden onset of blindness
• Swelling of the optic nerve
• Vision does not get better over time

Warnings in Wegovy Prescribing Information

The Prescribing Information for Wegovy includes information about sight-threatening eye disorders. Low blood sugar may cause blurry vision and other symptoms. The label also warns patients: “Tell your healthcare provider if you have changes in vision during treatment with WEGOVY.” However, it does not include warnings about the potential risk of NAION, permanent vision loss, or blindness.
