What is Proquin XR?

Proquin XR is an extended-release antibiotic that contains ciprofloxacin. It is prescribed to treat bacterial infections and prevent anthrax.

Proquin “Black Box” Warning

Like all fluoroquinolone antibiotics, Proquin XR has a “Black Box” warning label about tendonitis, which involves swelling of the fibrous tissue that connects bone and muscle. It also increases the risk of a tendon rupture during treatment and for several months afterward. These problems may affect tendons in the shoulder, hand, back of the ankle, and other parts of the body.

Proquin and Aortic Aneurysms

Tendon side effects occur because Proquin can damage collagen in connective tissue. Collagen is also found in the wall of the aorta, and the following studies suggest it may increase the risk of aortic aneurysms:

  • British Medical Journal (November 2015): Fluoroquinolone antibiotics triple risk of aortic aneurysms, based on data from 650,000 adults in Canada on antibiotics, including about 18,500 who had an aortic aneurysm.
  • JAMA Internal Medicine (October 2015): Current use of fluoroquinolone antibiotics was linked to a 2.4-fold increased risk of aortic aneurysm. Past use was linked to a 50% increased risk.

What is an Aortic Aneurysm?

The aorta is a major blood vessel that carries oxygen-rich blood from the heart into the body. It originates in the left ventricle and extends downward into the abdomen. An aneurysm occurs when the aorta enlarges and “balloons” outward due to a weak spot in the wall.

Proquin and Peripheral Neuropathy

In August 2013, the FDA issued a Safety Communication about the risk of peripheral neuropathy — nerve damage in the arms and legs — due to fluoroquinolone antibiotics. Peripheral neuropathy has been on the label for antibiotics since 2004, but the FDA updated the label to emphasize that nerve damage can sometimes occur rapidly (within days of the first dose) and cause permanent numbness, muscle weakness, tingling, burning, and problems feeling texture or temperature.

Studies of Fluoroquinolone Antibiotics and Nerve Damage

In August 2014, a study published in Neurology found that people who used fluoroquinolone antibiotics were twice as likely to develop neuropathy. In 2001, another study involving 45 cases of antibiotic-induced neuropathy found that many were “severe events that typically involved multiple organ systems” and 55% lasted more than one year.
