Studies Link Levaquin and Aortic Aneurysms
A study published in the British Medical Journal in November 2015 linked the use of fluoroquinolone antibiotics with a tripled increased risk of aortic aneurysms. Conclusions were based on data from 650,000 adults in Canada who took an antibiotic between 1997 and 2012, including nearly 18,400 people (1.1%) who had an aortic aneurysm.
In October 2015, JAMA Internal Medicine linked quinolone antibiotics with a doubled increased risk of aortic aneurysm. Individuals who were currently taking quinolone antibiotics were 2.4-times more likely to develop aneurysms. Past use of quinolone antibiotics increased the risk of aneurysm by 50%.
Conclusions of the study were based on data from nearly 150,000 people in Taiwan who used fluoroquinolone antibiotics between January 2000 and December 2011, including 1,477 individuals who experienced aortic aneurysm.
What is the problem?
An estimated 15,000 Americans die from aortic aneurysms every year. Patients with genetic conditions affecting connective tissue are more likely to develop aneurysms.
Levaquin has been associated with several other collagen-related side effects, including tendon ruptures and retinal detachment. The FDA added a Black Box warning about tendinitis and tendon ruptures in 2008, but has not issued warnings about aortic aneurysms.
The researchers warned:
“Not only is tendon composed of collagen, collagen is also a major extracellular matrix component of the aortic wall. … As fluoroquinolones may induce degradation of collagen causing tendinopathy, this raises the concern that fluoroquinolones may cause or aggravate aortic aneurysm and dissection by a similar mechanism.”
What is an Aortic Aneurysm?
The aorta is the main artery leaving the heart. In some cases, a weak spot develops on the wall of the aorta and balloons out. The stretched and bulging section is called an aneurysm, and it can burst and cause severe bleeding or death.
Aneurysms can become large before causing any symptoms. The symptoms also depend on whether the aneurysm occurs in the chest or abdomen. In both cases, severe internal bleeding can cause shock, a condition in which blood pressure drops so low that vital organs start shutting down.
Abdominal aneurysms usually develop slowly over many years and do not cause symptoms unless they rupture. These symptoms may include:
- Throbbing feeling in the abdomen
- Deep pain in the back or the side of the abdomen
- Steady pain in the abdomen that lasts for hours or days
Thoracic aneurysms in the chest can cause pain in the jaw, neck, back, or chest. Individuals may also have coughing, hoarseness, shortness of breath, or trouble breathing and swallowing. Ruptured aneurysms can cause sudden, severe pain in the upper back or abdomen.
Small aneurysms may not need treatment if they are not causing symptoms. However, aortic aneurysms are usually treated with medicines to lower blood pressure and relax blood vessels to reduce the risk of rupture. In serious cases, aneurysms require surgical treatment to remove the bulging section, implant a graft, or repair valves. In less-serious cases, the aneurysm isn’t removed and a doctor uses non-surgical techniques to insert a graft and strengthen the aorta.
