What is the Internal Bra?
The “internal bra” is a new type of plastic surgery in which doctors use mesh slings under the breast to add support for artificial implants.
Breast mesh is a flexible sheet of material that is shaped like a sling or internal “demi-cup” bra to support breast tissue or implants. Absorbable mesh is supposed to dissolve in the body over 6-24 months, leaving behind a natural scaffold of scar-tissue and collagen.
What is the risk?
There are many potential complications when mesh is used in breast surgery, for example:
- Infection
- Chronic pain or discomfort
- Nerve damage (may result in numbness, loss of sensation, or loss of intimacy with partners)
- Breast reconstruction failure
- Breast asymmetry
- Mesh migration, hardening, or detachment
- Interference with mammograms or other imaging scans

Types of Breast Mesh Surgeries
Surgical mesh implants that were originally designed and FDA-approved for hernia repairs and soft-tissue surgeries have been repurposed for a variety of unapproved “off-label” breast surgeries.
Breast mesh surgeries include:
- The “internal bra” (a type of plastic surgery)
- Implant-based breast reconstruction after a mastectomy
- Breast lifts (also called “internal mesh sling breast lift”)
- Breast augmentation (using mesh to support the weight of an artificial implants)
Mesh for Hernia Repairs Now Being Used in Breast Surgery
Surgical mesh that was originally approved for hernia repairs has been repurposed for breast surgeries. This is known as an “off-label” or unapproved use. The FDA has not approved any type of mesh for breast surgeries, but many doctors are now adding mesh when they perform a breast reconstruction, breast augmentation, or breast lift.
Why Do Doctors Use Mesh?
Doctors may claim that breast mesh will help provide more “natural-looking” or “longer-lasting results,” but these claims are not based on rigorous studies or the FDA. Instead, women should be aware of the risk of severe side effects, unproven results, and high expense.
No Long-Term Studies Or Clinical Trials
Breast mesh was never approved or evaluated by the FDA, so there have been no rigorous clinical trials or studies that have fully investigated the safety risks, benefits, and long-term outcomes.
Are Doctors Fully Informing Patients About the Risk?
Doctors are supposed to use their medical expertise to make decisions about what is best for each patient, which is why they are allowed to use surgical mesh in an unapproved “off-label” surgery like the internal bra. They also have a duty to fully inform patients about the risk of side effects, and the lack of FDA approval for breast mesh.
Thousands of Mesh Lawsuits Have Already Been Filed
Thousands of lawsuits have already been filed by people who were injured by mesh implants, such as hernia mesh or vaginal mesh, so doctors are well-aware of the potential risks. FDA Warnings for FlexHD and AlloMax Breast Mesh
The FDA has published several warnings about potential safety risks when mesh products are used in breast surgeries.
In March 2021, the FDA published a Safety Warning about higher rates of serious complications from two types of breast mesh products:
“The analysis showed significantly higher complication rates of explantation (implant removal), reoperation and infection in patients with FlexHD and AlloMax brands of ADM two years following surgery, when compared with patients who received SurgiMend or AlloDerm brands, or no ADM.”
FDA Warning for GalaFLEX and Phasix Breast Mesh
In November 2023, the FDA sent a letter to health care providers, reminding them that “the safety and effectiveness of surgical mesh in breast surgery, including in augmentation or reconstruction, has not been determined by the FDA.”
The FDA also updated the label on GalaFLEX and Phasix Mesh products to ensure doctors are aware that “surgical mesh products, including BD’s mesh products, have not been determined by the FDA to be safe and effective for use in breast surgery.”
The warning involves breast mesh products that are manufactured by BD (Becton Dickinson), including:
- Phasix Mesh
- Phasix ST Mesh
- Phasix Plug and Patch
- Phasix ST Mesh with Open Positioning System (OPS)
- Phasix ST Mesh with Echo 2 Positioning System, and
- GalaFLEX Lite Scaffold
- GalaFLEX Scaffold
- GalaFLEX 3D Scaffold
- GalaFLEX 3DR Scaffold
