Avid Medical Convenience Kit Recall
In April 2021, the FDA and Avid Medical announced a Class 1 recall for 1,570 medical convenience kits containing the recalled Chloraprep™ 3 mL applicators.
The kits contain Chloraprep, which is used to sterilize the skin of a patient undergoing dialysis, surgery, and other routine medical procedures. The products include:
- Adult Chest Tube Tray
- Fistula On-Off Kit
- Fistula (On/Off All in One Bag)
- Dialysis Access
What is the Problem?
BD/CareFusion recalled Chloraprep™ 3 mL applicators due to a risk of severe, life-threatening fungal infections with Aspergillus penicillioides. The manufacturer did not report any deaths, infections or injuries. Instead, the recall was issued after fungal growth was found in ChloraPrep.

ChloraPrep Recall
In June 2020, BD/CareFusion issued a limited recall for ChloraPrep™ 3 mL applicators in tropical environments with high heat and humidity.
In March 2021, under pressure from the FDA, BD/CareFusion decided to expand the ChloraPrep recall nationwide due to a risk of severe, life-threatening fungal infections.
Fungal Contamination Linked to High Heat and Humidity
The recalled ChloraPrep™ 3 mL applicators have a defective lid that may allow microbial growth if the product is stored in high heat and humidity (at least 86ºF and 75% relative humidity). If a contaminated product is used on a patient, it may lead to a severe fungal infection.
What is ChloraPrep?
ChloraPrep is used to sterilize the skin of a patient who is undergoing dialysis, surgery, catheter insertion, or orther medical procedures. The recalled product was distributed globally and is labeled with “CareFusion, El Paso, TX.” In the U.S., the recall includes:
- 260400 — ChloraPrep® One-Step 3 mL Applicator – Clear
- 260415 — ChloraPrep® One-Step 3 mL Applicator – Hi-Lite Orange
- 930400 — BD ChloraPrep™ Clear 3 mL Applicator
- 930415 — BD ChloraPrep™ Hi-Lite Orange™ 3 mL Applicator
