Midwest Folding Products Stage and Riser Caddy RecallSeptember 20, 2012 — The U.S. Consumer Products Safety Commission (CPSC) and Chicago-based company Midwest Folding Products are announcing a voluntary recall of stage and riser caddies, which caused the death of a 3 year-old girl last year. The problem is that the caddy does not have an automatic locking mechanism to secure stages and risers when they are stored on the caddy. The heavy, unsecured platforms can potentially fall and cause severe injuries, especially for young children.

The 3 year-old girl died in May 2011 at a church in Greenville, South Carolina. She suffered severe head trauma when part of a portable stage fell on her from the unsecured caddy.

Midwest Folding Products is recalling the caddies. The products are 44 inches long, 30 inches wide, and 60 inches high. They may be either black, brown, or gray metal. The products were sold from January 1989 until March 2012. Approximately 5,300 caddies are involved in the recall. The products were sold to churches, schools, performing arts centers, and other organizations throughout the United States. They cost about $1,200.

Midwest Folding Products is in the process of notifying known customers who have purchased the products. Customers should check whether the stage/riser caddy at their organization is involved in the recall.

Customers who own the caddies should stop using the products immediately, and contact Midwest Folding Products for a free repair kit. The kit will include new brackets and latches that automatically secure stages and risers in the caddy. The repair kit will also include new warning and instruction labels.
