Cook Celect IVC Filter Linked to 43% Perforation RateAugust 10, 2015 — Researchers comparing two retrievable IVC filters have found a 43% rate of perforation associated with the Cook Celect compared to a 0% rate associated with the Rex Medical Option filter.

The study was published in the Journal of Vascular Interventional Radiology in June 2015.

During a 33-month period at one hospital, 99 Cook Celect filters and 86 Option filters were implanted. On average, they were retrieved after two months. Retrieval rates were not significantly different, but the Option did require significantly more fluoroscopy time and adjunctive retrieval techniques.

Unfortunately, the incidence of strut perforation was 43% for Celect filters versus 0% for Option filters. The struts are straight wire legs that anchor the filter to the walls of the vena cava.

Other studies have also found high rates of perforation associated with the Celect filter. One study published in April 2012 found that after 71 days, varying degrees of perforation occurred in all 27 patients who were implanted with the Celect.

In another study published in 2009, 115 people were implanted with the Cook Celect IVC filter. In some cases, doctors were unable to retrieve the filter because it had become embedded in the endothelial wall of the vena cava.

Perforation of the vena cava does not always cause symptoms or problems. However, there are cases when the perforation damages surrounding organs — such as the aorta, intestines, ureter, or abdomen. In general, studies have estimated the rate of perforation at 9-25%, which suggests the Cook Celect might have a higher risk of perforation than other filters.
