June 25, 2012 — Approximately 2.3 million Sloan Flushmate III Pressure-Assist Flushing Systems are being voluntarily recalled because they could explode, causing serious personal injury and property damage. The Consumer Products Safety Commission (CPSC) is aware of at least 304 cases in which the toilets have exploded. At least 14 people have had “impact and laceration” injuries, though the CPSC did not describe how serious the injuries were.

 

The Flushmate III is made of two pieces of black plastic, which are installed in the toilet tank. According to the CPSC, the devices are prone to failure because a weld is unable to withstand the intense pressure from air and water. The sudden release of pressure can lift the lid off the toilet tank, shatter the toiled, and cause an injury.

Customers can identify the recalled toilet via the “series number” of 503, or from the 16-digit serial number. The first six digits of the serial number are the unit’s manufacturing date. The defective products were manufactured between October 14, 1997 and February 29, 2008.

The CPSC is advising that all people who own the Sloan Flushmate III should stop using the product. Water should be disconnected from the device so it cannot be used. The product should be uninstalled from the toilet or repaired.

The product can be repaired, and Flushmate is offering customers a free repair kit. Customers may get the kit from the company’s website, recall.flushmate.com, or by calling a toll-free hotline at (800) 303-5123.

The Flushmate III may have been purchased at a Home Depot, Lowe’s, or other hardware store. The products were sold by several toilet manufacturers, including Gerber, Mansfield, Kohler, St. Thomas, American Standard, Crane, and Eljer.
