Testim and Venous Thromboembolism (VTE)

Testim is a testosterone replacement gel that is known to increase the risk of blood clots in the legs. The Prescribing Information includes warnings about this side effect, which is also known as deep vein thrombosis (DVT).

In July 2014, the U.S. Food and Drug Administration (FDA) published a Safety Communication to announce that they would require warnings about venous thromboembolism (VTE) on the label for Testim.

According to the FDA:

“There have been postmarket reports of venous blood clots unrelated to polycythemia … Because these clots occur in the veins, this new warning is not related to FDA’s ongoing evaluation of the possible risk of stroke, heart attack, and death in patients taking testosterone products.”

What is Venous Thromboembolism?

Venous thromboembolsm (VTE) or “venous thrombosis” occurs when blood clots in the legs break loose and travel in the bloodstream (called an “embolism”). Blood clots travel until they get trapped inside smaller blood vessels in the lungs, causing a pulmonary embolism (PE), which is like a heart attack in your lungs. The chain of events involving DVT and PE is known as a VTE.


DVT does not always produce symptoms in its early stages. If symptoms do appear, they may include:

  • Swelling in the affected leg, including ankle and foot
  • Skin discoloration (redness, paleness)
  • Skin feels warm to the touch
  • Pain, discomfort, or tenderness in the leg or calf

If blood clots reach the lungs, you may experience the following symptoms of a PE:

  • Sudden shortness of breath
  • Chest pain or discomfort that worsens with deep breathing
  • Feeling dizzy or light-headed
  • Rapid heartbeat
  • Coughing up blood
  • Sweating
