What is Proscar?

Proscar is a prescription medication that contains 5-mg of finasteride, a 5α-reductase inhibitor (5ARI). It was developed by Merck & Co. and has been approved by the FDA since 1992. It is indicated for the treatment of benign prostatic hyperplasia (BPH) in men with enlarged prostate. Merck also sells a 1-mg dose of finasteride as Propecia, a hair-loss treatment.

Proscar and Gynecomastia (Male Breasts)

According to the Prescribing Information, more than 1% of men on Proscar develop breast enlargement and/or breast tenderness. These are the most common symptoms of gynecomastia, a disfiguring and embarrassing medical condition involving the enlargement of the mammary gland in the male breast.

Gynecomastia is different from male breasts caused by fatty deposits or obesity. Instead, it involves fibrous glandular tissue that cannot be removed with liposuction. Surgical excision of the gland is necessary to treat severe gynecomastia.

Proscar and Sexual Side Effects

Proscar decreases levels of dihydrotestosterone (DHT), which can lead to higher levels of estrogen and other hormonal imbalances. One of the most common side effects is sexual dysfunction, including impotence (erectile dysfunction), decreased libido, decreased volume of ejaculate.

According to clinical studies of Proscar, about 2% of men reported sexual dysfunction, but these problems resolved in patients who stopped taking finasteride. Unfortunately, recent studies and case reports suggest that sexual dysfunction may persist for months, years, or indefinitely in some men who discontinue Proscar.

FDA Warning

In 2011, the label on Proscar was updated to include erectile dysfunction that did not improve after the drug was discontinued. In April 2012, the FDA announced more changes to expand the list of sexual side effects that were reported to persist after Proscar was discontinued.

After reviewing hundreds of adverse event reports, the FDA warned that persistent sexual side effects of Proscar could include decreased libido. They also warned about reports of poor semen quality and male infertility that normalized or improved after Proscar was discontinued.

According to the FDA Safety Warning:

“Sexual adverse events associated with Proscar use were identified in two clinical trials. This information is included in the Proscar label.  In one trial, these events were reported more frequently during the first year of treatment with Proscar as compared to men who received placebo.  In years 2-4 of the trial, there was no significant difference between treatment groups in the incidences of impotence, decreased libido and ejaculation disorder.”
