How Does Sitagliptin Treat Type-2 Diabetes?

Janumet XR contains sitagliptin is a new drug in a new class of drugs called “incretin mimetics.” These drugs work by mimicking incretins (digestive hormones that regulate insulin and blood-sugar levels). Sitagliptin works by inhibiting an enzyme called DPP-4 (dipeptidyl peptidase-4). Normally, DPP-4 breaks down an incretin hormone called GLP-1 (glucagon-like peptide-1). Without DPP-4, more GLP-1 stimulates the pancreas to make more insulin, which lowers blood-sugar.

Janumet XR and Pancreatitis

Sitagliptin works by directly targeting cells in the pancreas to make more insulin. Many experts are concerned that this could lead to inflammation in the pancreas (pancreatitis). In August 2009, the FDA issued a Drug Safety Communication to report 88 cases of acute, hemorrhagic and necrotizing pancreatitis in patients on sitagliptin.

Janumet XR and Pancreatic Cancer

Chronic pancreatitis is a major risk-factor for pancreatic cancer. In March 2013, the FDA issued a Drug Safety Communication after reviewing evidence linking sitagliptin and pre-cancerous pancreatic growths. The study, published in the journal Diabetes, found evidence that exposure to sitagliptin could cause excessive cell proliferation in the pancreas. Autopsies showed that pancreases were ~40% larger than normal, with 6-times as many beta-cells. Three pancreases had micro-adenomas, and one pancreas had a neuroendocrine tumor.
